An immunocompetent patient living in an endemic area presented with headache, weakness, gait instability, and weight loss. Brain imaging showed enhancing lesions, but fungal infection was not initially suspected. This author reminds clinicians to keep histoplasmosis in the differential diagnosis when enhacing lesions are identified.
