The American Cancer Society (ACS) has issued new guidelines that recommends colorectal cancer (CRC) screening to begin at age 45 years for average-risk adults. Similar guidelines from other organizations recommended beginning screening at age 50 years. The ACS used an existing systematic evidence review of the CRC screening literature and microsimulation modeling analysis, including a new evaluation of the age to begin screening by race and sex and additional modeling that incorporates changes in US CRC incidence. Results from modeling analyses identified efficient and model-recommendable strategies that started screening at age 45 years. Among other ACS recommendations:
- Regular CRC screening beginning at age 50 years is strongly recommended.
- Regular screening is recommended through age 75 years for those at average risk who have a life expectancy of >10 years.
- Clinicians should individualize the decision to screen for adults aged 76 to 85 years.
- CRC screening should be discouraged among those aged ≥86 years.
