The Advisory Committee on Immunization Practices (ACIP) has recommended Heplisav-B (HepB-CpG), a yeast-derived vaccine prepared with a novel adjuvant, for use in adults aged ≥18 years. The ACIP Hepatitis Vaccines Work Group conducted a systematic review of the evidence, including data from 4 randomized controlled trials assessing prevention of hepatitis B virus (HBV) infection and 6 randomized controlled trials assessing adverse events in adults. Among the findings and recommendations:
- Seroprotective antibody to hepatitis B surface antigen (anti-HBs) levels were achieved in 90.0% to 100.0% of those receiving HepB-CpG compared with 70.5% to 90.2% of subjects receiving Engerix-B.
- Heplisav-B vaccine is administered as 2 doses, 1 month apart.
- Those who should receive the vaccine include persons at risk for infection through sexual exposure, those with a history of current or recent injection drug use, and those at risk for infection by percutaneous or mucosal exposure to blood.
- Persons with hepatitis C virus (HCV) infection should also receive the vaccine, as should those with HIV, incarcerated persons, and international travelers to countries with high or intermediate levels of endemic HBV infection.
