Carbapenem Resistance in Healthcare Settings

Carbapenem non-susceptible (Carb-NS) rates were significantly higher in US intensive care units (ICU) in compared to non-ICU settings, a recent study found. Researchers evaluated the rates, infection sources, and pathogen types associated with Carb-NS Gram-negative isolates in ICU and non-ICU settings in a large US hospital database. They conducted a retrospective cross-sectional analysis of carbapenem susceptibility of all non-duplicate isolates of Gram-negative pathogens collected from January 1, 2017 to December 31, 2017 at 358 US hospitals. Among the findings:

  • The Carb-NS rate for 312,075 Gram-negative isolates was 3.4% and >half were Pseudomonas.
  • The highest rates of Carb-NS were found in Acinetobacter spp. and P. aeruginosa.
  • The rate of Carb-NS was significantly higher in ICU vs non-ICU settings (5.4% vs 2.7%) and remained significant in multivariable analysis after adjusting for infection and hospital characteristics.
