PACT-HF: Transitional care derives no overall benefit

Key clinical point: Transitional care in heart failure did not improve outcomes compared to usual care.

Major finding: Rates of all-cause death, readmissions, or ED visits were 63.1% for transitional care and 64.5% for usual care (P = .50).

Study details: Patient-Centered Transitional Care Services in Heart Failure (PACT-HF) randomized clinical trial of 2,494 HF patients in Ontario.
